1 Full-Time Permanent role: 35 Hours per Week
Reason for Posting: An existing vacancy.
Salary Range: $56,444--$68,429 per annum *This represents steps 1-7 of a seven-step
salary grid. Candidate experience and internal equity will be considered when
determining the starting salary
Location: This Case Manager role will primarily work in our Newmarket Court location
located at 50 Eagle Street West Room 2088 Newmarket, ON, CAN and Newmarket
office at 1101 Nicholson Road Unit 5 Newmarket, ON and will be required to travel
within York Region & South Simcoe.
Job Profile:
The Case Manager – Mental Health and Justice Program assists adults and youth
(aged 12 years and older) who have the prospect of having a mental illness or dual
diagnosis (mental health and developmental disability, traumatic brain injury, or
concurrent disorder) and have involvement in the criminal justice system.
The Case Manager collaborates with individuals to identify strengths and needs and
develop long-term goals, short-term objectives to meet these goals, and concrete action
steps to achieve the objectives.  As part of this process, individuals are assisted in
acquiring skills and connecting with formal and informal support, peer support,
professional services, and resources to reduce the likelihood of further conflict with the
law, enable progress toward recovery, and improve quality of life.  Where required,
referrals are made to longer-term services.
The Mental Health and Justice Case Manager will also assist individuals and their
supports in navigating the court system and offer support through information, referral,
and consultation. The Case Manager will work directly with court staff to explore

possible outcomes through the court process by assisting the clients address the
correlation between the offense and their Mental Health needs.
In addition to compliance with all organizational policies and procedures, the incumbent
is also expected to behave ethically, demonstrate competence in effective
communication and team building, demonstrate effectiveness in working in a
collaborative environment, and demonstrate confidence in decision-making and in
building relationships.
This position reports to the Manager, People, and Teams
Requirements:
 Degree or diploma in health or social services
 Degree plus four years or more experience in a community-based mental-health
setting, experience with the criminal justice system is an asset
 Diploma plus six years or more experience in a community-based mental-health
setting, experience in mental health diversion is an asset
 Current registration with the OCSWSSW or a regulatory body under the
Regulated Health Professions Act
 Knowledge of systemic issues and social determinants of health such as poverty,
unemployment, stigma, and the isolation felt by individuals with serious mental
illness and their families
 Knowledge of the Ontario Mental Health Act and mental health reform principles,
the Substitute Decisions Act, the Health Care Consent Act, and PHIPA
requirements
 Ability to apply Recovery principles and empowerment-oriented philosophies and
practices in work with consumers
 A second language reflecting the local community is an asset
 A valid Canadian driver’s license, a minimum of $1,000,000 third-party liability
insurance, and proof that insurance covers the use of a personal vehicle to
transport clients
 A satisfactory Vulnerable Sector Screening (Police Check)
Working Conditions: Office work, community settings, meetings in clients’ homes and
travel with clients.
