We are hiring a Housing Support Workers for the Community Homes for
Opportunity (CHO) Program!
If you are a collaborative, innovative individual who is interested in helping people
recover from a mental health challenge, increase their independence, and find housing
in York Region and South Simcoe then this role is for you!
Position: 1- year Contract, full-time, 35 hours per week. - Evening hours and weekends
may be required for both positions.
Salary: $56,444-68,429 per annum.   *Candidate experience will be considered when
determining the starting salary.

Location: The Housing Support Worker will work predominantly in the community,
directly with clients. The Housing Support Worker will work from the Aurora Office
located at 15150 Yonge Street on the days of on-site work and will be required to travel
within York Region & South Simcoe for client meetings and staff training or other
meetings. There is potential to work from home based on client and program needs.
Job Profile:
The Community Homes for Opportunity Housing Support Worker works within the
community in residential congregate living environments and collaborates with clients to
provide individualized and flexible support and wellness planning.  The focus is to help
clients maintain their housing, engage and integrate them into the community, budgeting
and financial management support and helping to facilitate access to resources and
supports to maximize their independence and potential, and promote their recovery.
Individuals are assisted to identify strengths and needs and develop long term goals,
short term objectives to meet these goals, and concrete action steps to achieve the
objectives through wellness plans. In addition, the Community Homes for Opportunity
Housing Support Worker participates in individual and systemic advocacy initiatives to
improve resource access and availability.  The Community Homes for Opportunity
Housing Support Worker may be assigned other duties as required and will work very
closely with the CHO Housing Administrators and Home Operators.  They are part of a
multi-disciplinary team.
In addition to compliance with all organizational policies and procedures, the incumbent
is also expected to behave ethically, demonstrate competence in effective
communication and team building, demonstrate effectiveness in working in a
collaborative environment, and demonstrate confidence in decision-making and in
building relationships.
Requirements:
 A degree or diploma in health or social services
 Current Registration with the Ontario College of Social Workers and Social
Service Workers (OCSWSSW)
 Degree plus four years or more experience in a community-based organization
working with adults with serious mental illness in the community
 Diploma plus six years or more experience in a community-based organization
working with adults with serious mental illness in the community

 Experience working with people in congregate living environments, Homes for
Special Care or Housing with Supports is preferred
 Experience working with individuals experiencing serious mental illness,
concurrent disorders and dual diagnosis (developmental disability and/or
traumatic brain injury) is preferred
 Knowledge of the Ontario Mental Health Act, mental health reform principles, the
Substitute Decisions Act, the Health Care Consent Act and PHIPA requirements
 Ability to apply Recovery principles and empowerment oriented philosophies and
practices in work with clients
 A second language (French, Cantonese, Farsi, Italian, Korean, Mandarin,
Punjabi, Russian, Tamil, Urdu) reflecting the local community is preferred
 A valid Canadian Full G driver’s license, a minimum $1,000,000 third party
liability insurance and proof that insurance covers use of personal vehicle to
transport clients
 A satisfactory Vulnerable Sector Screening (Police Check)
